Mental Health Practitioner: Rule 79 Adult Mental Health Targeted Case Manager

Targeted Case Management is a program designed to help adults with Serious and Persistent Mental Illness diagnoses (SPMI) to access services, and to stay living as independently as possible in their communities. Case managers help through referral, coordination, advocacy, resource linkage, and ongoing assessment of individual functioning and needs. Case managers visit clients in the community (home, library, community room, etc) to assess their needs and help them get connected to supports and resources for stabilization and ongoing care. Targeted Case Management is an awesome position to gain experience working with clients who have serious mental health diagnoses, and to learn client systems, from the individual to familial to the societal and macro levels. This is an exciting job allowing diversity of day-to-day experiences, some in office and some in the community. We believe this is an excellent position for anyone seeking to develop their skills and knowledge base in the field of mental health.
